Midian was the region east or southeast of the Sinai Peninsula where Moses had lived in exile; Egypt was the land of Israel’s oppression, to which God now sent him back because those seeking his life had died. The Nile, though central to Egypt’s life and later plagues, is not mentioned in Exodus 4:19 itself.
